Referrals to Pact services can be made by individuals, whanau, needs assessors, health professionals or other community services. All people, however, must have a needs assessment prior to entering Pact services.

A needs assessment is the first step in identifying what help you need to manage as independently as possible. A needs assessor will meet with you, your family/whanau (if appropriate) and anyone else you may wish to have present (for example, a support person). Together, you will identify the areas in which you need assistance. This meeting will take place somewhere that is comfortable for you - usually your own home. The resulting Needs Assessment Report will outline your desired outcomes and the range of services that may meet your needs.
